Contact and assessment inquiry forms request basic details such as your name, email, contact preference and a brief administrative message. Please do not include diagnoses, medical histories, testing results or other sensitive clinical information. Use the clinical process provided by the practice for those materials.
How website information is used
Information submitted through this site is used to respond to your request, review the appropriate administrative next step, arrange appointments and maintain scheduling history. Sending a message or scheduling a consultation does not establish a therapeutic relationship.
Website technology
WordPress uses cookies for functions such as authenticated access. Hosting systems may record technical information such as an IP address, browser information and requested pages to operate and protect the site. The contact form uses a temporary hashed network identifier to limit repeated submissions. The site loads fonts from Google, which receives the technical information needed to deliver those files.
Scheduling and outside services
Scheduling, calendar, payment, email and telehealth providers may process information needed to deliver their services when those integrations are enabled. Appointment availability shown publicly should not reveal private calendar details. Clinical forms and records belong in the practice’s designated clinical system. Access, retention and questions
Administrative access is restricted. Information is retained as needed for the purpose for which it was collected and applicable recordkeeping obligations; deleting website data may not permit deletion of records the practice must keep. Internet communications cannot be guaranteed risk-free. To ask about information you have submitted or request a correction, use our contact page and request a privacy-related response without including clinical detail.
Updates
This policy will be reviewed when the site’s tools or practices change. The practice will confirm its production providers, retention schedule and effective date before public launch.
